If a loved one has had an accident, or a medical event and subsequently had a Needs Assessment that determines they should not return to living at home, and must find Rest Home, Hospital or Dementia Level Care, we can help.
We understand that some families are not immediately available. You could be living overseas, in other parts of the country, or you may not have the time or resources to assist immediately. If this is the case, please get in touch so that we can be your partner on the ground. We will talk to you and your family, and we will endeavour to find the perfect new home. Additionally, we will assist with the full admission process, all with the intention of reducing stress on you and your family.
If you need, we can then partner with family to downsize belongings, sell vehicles or assets, and prepare property for sale. We also work with local Real Estate agents that we can recommend and arrange for you.


Finding the right Rest Home
Tell us your priorities of what you would like in a Rest Home / Care Facility, and we will do the homework for you, compile a list of available facilities, and even inspect them with you, to assist in making your choice.
We can then assist with the admission, subsidy applications, and facility contracts, to make sure everyone understands the finer details with clarity.
Our Placement Advocacy Process: Step-by-Step
Finding the right care facility is a journey. Whether you are across the street or across the world, we act as your local expert, advocate, and hands-on support system to ensure a seamless transition.
Phase 1: Discovery & Strategy
- Step 1: Initial Consultation We start with a deep-dive conversation to understand the senior’s specific needs, medical requirements, and lifestyle preferences. We discuss location, budget, and the “must-haves” that will make a facility feel like home. We can then provide a fixed price for our services, so you know exactly what to expect.
- Step 2: The Search & Narrowing Down Using our local knowledge, we conduct an exhaustive search for available beds in the Canterbury region. We filter these based on your criteria, presenting you with a curated shortlist of the best-fit options.
Phase 2: Inspection & Decision
- Step 3: Professional Inspections We visit the facilities on your behalf (or with you). We don’t just look at the décor; we check the “feel” of the home. We take detailed videos and photos of the actual rooms available and key areas of the facility so you can see exactly what we see.
- Step 4: The Review Meeting We meet with the family—either in person or via an online video call—to go over our findings. We answer your questions, compare the options side-by-side, and help you reach a confident decision on the best placement.
Phase 3: Admission & Logistics
- Step 5: Securing the Room Once the choice is made, we move quickly to secure the room and manage the initial admission arrangements with the facility management, and any other stakeholders.
- Step 6: Paperwork & Move-In Support The day of the move can be the most emotional part of the process. We are there on-site with the senior, ensuring the room is set up correctly and providing a familiar, supportive face as they settle into their new environment.
Phase 4: Settling In (The First 14 Days)
We don’t just “drop and go.” The first two weeks are critical for a successful transition.
- The Follow-Up (Day 1): We visit the very next day to see how the first night went and address any immediate concerns.
- Ongoing Monitoring (Days 4, 7, 10): We return every 3–4 days for the first fortnight. This allows us to spot and fix any “teething issues”—from meal preferences to care routines—ensuring the senior feels heard and the family stays informed.

Senior Transitions offer Chaperone services for older adults. We can attend Medical or Legal appointments with with Seniors to ensure the right information is received, understood, and shared with appropriate family.

I wish there was an option for 10 stars. I actually don’t know what we would have done if we didn’t find Jeremy from Senior Transitions. The road ahead when we landed back in NZ to help our Dad after his stroke was so daunting. Jeremy made this such and easy process, such a nice person, nothing was a problem, he helped us with a timeline and executed on each step. I actually can’t believe we got his care, house, belongings and everything in between sorted in the limited time we had in Christchurch. Our dad is in a great home and our stress levels are now very minimal and we can enjoy our time with him.
